Local Sacramento hospital is in need of an Inpatient Staff Pharmacist for a full time, permanent position.
JOB SUMMARY: The Pharmacist is responsible for providing day-to-day comprehensive, patient oriented pharmacy services in any or all areas of the department. The pharmacist must perform effectively and maintain competency in clinical, professional, managerial and operational disciplines. The staff pharmacist is responsible for fulfilling the departments mission to provide high quality, cost effective, innovative pharmaceutical care to patients.
Collaborates with other health care professionals to manage patient’s total drug therapy for effectiveness, side effects, adverse drug reactions, toxicities, drug interactions, and incompatibilities following guidelines for formulary management and clinical intervention tools.
Identifies and follows up to resolve drug related problems to maximize patient outcomes for all patients.
May rotate on the I.V. Admixture Service; performs interim and final check(s) for accuracy, completeness and quality while ensuring compliance with all USP 797 regulations for compounded sterile products preparation
Manages inventory to minimize waste and expired drugs in the work area while maintaining sufficient inventory level to avoid shortages
Manages daily workload fluctuations with respect to staffing assignments and work output. Oversees activities of technical staff assigned in the area. Coordinates activities with technical staff to ensure accurate and timely delivery of medications to patient care areas and quality management duties.
Processes all orders/requests accurately and efficiently, verifying appropriateness of all drugs in reference to the patient’s diagnosis, allergies, height weight, and other relevant lab data. Verifies/Confirms technician order entry and order filling/preparation prior to dispensing.
Active California Pharmacy License
1+ years of inpatient experience or a clinical PGY-1 residency
