Duties:

• Collaborate with hematologists/oncologists, advanced practice providers, and other clinical staff to provide patients with individualized treatments utilizing the most recent and relevant evidence.
• Review chemo- and immunotherapy orders for accuracy, appropriateness, and completeness.
• Evaluate and interpret drug related laboratories. Recommend dose adjustment, addition, or discontinuation of medication(s) as appropriate.
• Identifies, resolves, and prevents medication-related problems, including, subtherapeutic dosage, overdose, drug-drug interactions, drug-disease interactions, adverse reactions, inaccuracies, and inappropriateness.
• Drug utilization management.
   

Qualifications:

• CA Pharmacist License (in good standing)
• Extensive Oncology Experience OR
• PGY1/ PGY2 Oncology Residency

Location: Los Angeles, CA
Duration: Temp to direct hire or direct hire
Pay Rate: $130,000- $150,000 depending on experience
