Position Summary:

As a Clinical Pharmacist, you will play a vital role in delivering comprehensive and patient-centered pharmaceutical care. In this position, you will be responsible for medication therapy management, patient counseling, interdisciplinary collaboration, medication safety initiatives, and continuous education. Your expertise will be utilized to optimize drug therapy outcomes and ensure the highest standard of care for our valued patients.

Responsibilities:

- Conduct comprehensive medication therapy management, including medication review, therapeutic interventions, and patient counseling
- Collaborate with interdisciplinary teams to develop and implement individualized treatment plans
- Actively participate in medication safety initiatives, including medication error reporting, monitoring adverse drug reactions, and implementing appropriate interventions
- Provide drug information and education to healthcare professionals, patients, and their families
- Stay updated on the latest developments in pharmacotherapy and contribute to the implementation of evidence-based practices
- Perform clinical rounds and consult with physicians and other healthcare professionals to optimize patient care
- Monitor and adjust medication regimens to achieve desired outcomes while minimizing adverse effects
- Provide guidance and mentorship to pharmacy technicians and support staff

Qualifications:

- Current Montana pharmacist license (required)
- Prior experience as a Clinical Pharmacist or in a similar clinical role (required)
- Strong clinical knowledge and understanding of evidence-based pharmacotherapy
- Excellent communication and interpersonal skills to collaborate effectively with healthcare teams and patients
- Detail-oriented approach and a commitment to providing accurate and safe medication therapy
- Ability to prioritize tasks and handle multiple responsibilities in a fast-paced environment
