Location: Sylmar, CA
Schedule: 8-hour shifts. Must be flexible to work all shifts
Pay Rate: $60.00/hr. + health benefits

Job Responsibilities:

• Participates in the development and continual monitoring of patient drug therapy treatment plans and patient outcomes in clinical specialty programs
• Performs patient assessments and monitors the effects of drug therapy using clinical symptoms, evaluating laboratory data, and other relevant clinical diagnostic techniques
• Attends daily rounds and medical conferences with physicians in specialty medical areas to evaluate patient drug therapy outcomes
• Interviews patients to obtain information regarding drug use, drug allergies and sensitivities, and medication histories; assesses compliance and counsels patients on appropriate drug use
• Interprets and applies pharmacokinetic drug data in recommendations for patient-specific drug dosage regiment
• Formulates individualized drug regimens based on patient specific parameters
• Advises and assists in implementing cost containment strategies for the usage of pharmaceutical products
• Participates in performance improvement activities to ensure optimal pharmaceutical care
• Dispenses medications as necessary

Qualifications:

• Registered Pharmacist issued by the California State Board of Pharmacy, active and in good standing
• Full time availability and flexibility in scheduling
• COVID Vaccine required, no exceptions
• Must have prior hospital pharmacy experience
