Full-Time 13-week Travel Clinical Infusion Pharmacist - ND Home Infusion Center

In this position, you will be responsible for:

• As a Clinical Home Infusion Pharmacist, you will review charts and lab results for an assigned caseload of patients receiving specialized, complex infusion therapy and TPN care
• You will communicate routinely with the patient and caregivers, answering questions, gathering information, and discussing updates and course recommendations with the physician
• Building strong one-on-one relationships with your patients while they are recovering or receiving routine treatments, and supporting remote patients on a routine schedule and as needed
• Collect, interpret, and evaluate all physician medication orders for therapeutic appropriateness, cost effectiveness, accuracy, safety, and completeness

Qualifications should include:

• Recent Clinical Home Infusion experience with familiarity in all areas
• PharmD preferred.  BS from accredited School of Pharmacy.  PGY1 and PGY2 residency candidates encouraged.
• Active/clear State Board of Pharmacy license
• Active ASHP approved IV Certification Highly preferred

Hours: 8 hour shifts between 7am-7pm, varied days with weekend rotation.
