Provide specialized physical therapy services to patients within their homes, addressing physical impairments, functional limitations, or disabilities stemming from injury, disease, or other causes. This role requires collaboration with patients, families, physicians, and allied health professionals to develop and coordinate effective care plans tailored to individual needs. Patients range in age from infants to elderly adults, requiring adaptability and a patient-focused approach.

Key Responsibilities:

  • Deliver individualized physical therapy treatments in patients' residences.
  • Assess patient needs and set rehabilitation goals in collaboration with interdisciplinary teams.
  • Maintain accurate, timely electronic documentation in compliance with agency policies and professional standards.
  • Travel to patient locations as assigned, ensuring consistent, quality care.
  • Participate in weekend rotations approximately once every six weeks.

Qualifications:

  • Current Massachusetts Physical Therapist professional license (mandatory).
  • Minimum of one year home care experience.
  • Strong communication and interpersonal skills to engage patients and interdisciplinary teams.
  • Ability and willingness to travel to multiple patient locations in the Cambridge, MA area.
