Physical Therapist Home Health - Newton Center, Massachusetts

Provide specialized physical therapy services to patients in their homes, supporting individuals with physical impairments, functional limitations, or disabilities caused by injury, illness, or other factors. This role requires collaboration with patients, families, physicians, and other healthcare professionals to develop and coordinate personalized care plans. Documentation and communication must be timely and thorough, adhering to agency policies and the standards of practice for home care physical therapy.

Qualifications and Desired Experience:

• Current Physical Therapist license in Massachusetts (required)
• Minimum of 1 year of home care physical therapy experience
• Comfortable with weekend rotations, approximately once every six weeks
• Ability and willingness to travel to patient homes as assigned
• Strong interpersonal and communication skills to work with interdisciplinary teams and patients of all ages, from infants to the elderly
