A rewarding opportunity is available for a Registered Nurse with expertise in Home Health and Hospice care to provide essential community-based nursing services. This contract role requires a compassionate and skilled RN to deliver personalized care and support to patients in their homes, promoting quality of life and comfort.

Key Responsibilities:

  • Provide skilled nursing care to patients in a home health and hospice setting.
  • Manage a caseload of 4–7 patients per day, depending on shift length.
  • Perform assessments, develop care plans, and administer treatments according to patient needs.
  • Collaborate with interdisciplinary teams to optimize patient outcomes.
  • Respond to on-call duties 2–4 weekday nights per week and 1–3 weekends during the assignment.
  • Support med/surg inpatient units as needed.

Qualifications and Desired Experience:

  • Minimum 2–3 years’ experience in Home Health nursing.
  • Hospice experience is preferred; training support available if needed.
  • Current BLS and ACLS certifications required.
  • Ability to work holidays such as Thanksgiving and Christmas is highly advantageous.
  • Must be committed to delivering compassionate, patient-centered care.

Schedule:

  • 40 hours per week, typically Monday through Friday.
  • 8-hour shifts preferred (5x8 schedule).
  • Opportunity to participate in call shifts and weekend coverage.
