Position Overview:
Soliant Health is currently seeking a skilled and motivated Hospital Pharmacist to join our team for a temporary assignment at a prestigious hospital in Portland, Oregon. This position offers a unique opportunity to work in a dynamic healthcare environment and make a meaningful impact on patient care.
Responsibilities:
- Review medication orders and ensure accurate dispensing and administration of medications.
- Collaborate with healthcare professionals to optimize medication therapy plans.
- Provide drug information and counseling to patients and healthcare providers.
- Participate in medication reconciliation processes.
- Perform medication-related quality improvement activities.
- Maintain accurate and complete documentation.
Qualifications:
- Active and unrestricted Pharmacist license in Oregon.
- Graduation from an accredited School of Pharmacy.
- Prior hospital pharmacy experience is strongly preferred.
- Strong knowledge of pharmacy laws, regulations, and best practices.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Strong attention to detail and problem-solving abilities.
- Flexibility to adapt to changing work environments and schedules.
