MTM Pharmacist - Full Time MN

Soliant Health has partnered with a growing medical facility and seeking a qualified MTM Pharmacist in Minnesota.

Responsibilities

• Perform comprehensive medication therapy sessions with MTM eligible members: review member’s medication profile to identify, prevent and resolve medication-related problems to help members maximize benefits and minimize cost; document the member encounter in the MTM platform
• Provide medication education to members regarding drug therapy, compliance with prescribed medications, appropriate use of medications, minimizing side effects, etc.
• Engage members in conversations related to medication adherence and improving health outcomes, and document findings
• Review potential drug to drug and drug to disease interactions identified in member’s profile
• Contact member’s physician as necessary to seek resolution on medication related problems, including opioid use concerns
• Contribute to projects as needed to support and improve the overall program
• Work with Pharmacy Technicians and Service Specialists to provide guidance and support
• Other duties as assigned

Minimum Qualifications

• Doctor of Pharmacy (PharmD) or Bachelor of Science Pharmacy Degree
• Current pharmacist license in good standing with the State Board of Pharmacy in the State where the position is located
• 1 year of clinical, retail, hospital, managed care or relevant experience
• Microsoft Excel and Word proficiency
• Must be eligible to work in the United States without need for work visa or residency sponsorship

• Analytical and problem-solving skills with attention to detail and quality
• Ability to manage time while working on multiple projects simultaneously
• Ability to develop process efficiencies
• Ability to work well in team environment

Preferred Qualifications

• Pharmacy Care/Case Management experience
• RxClaim experience
• MTM experience or completion of a pharmaceutical care training program
• Experience working with Medicare members
