Location: Houston, TX

Schedule: Variable days and afternoons, 8-hour shifts, rotating weekends

Job Responsibilities:

• Evaluate the appropriateness and effectiveness of patient’s active medication therapy
• Recognize untreated medical issues that could be improved or resolved with appropriate medication therapy
• Monitor patient progress for regimen effectiveness and safety
• Communicate effectively with physicians, nurses, and other healthcare providers to provide clinical pharmacy services and direct patient care activities
• Provide general clinical services such as pharmacokinetic based dosing, anticoagulant monitoring, pain management, nutrition support, IV to PO, renal dosing and other programs as defined by pharmacy leadership
• Demonstrate proficiency in clinical pharmacokinetic concepts and independently provide clinical pharmacokinetic services
• Demonstrate proficiency in anticoagulation therapy and independently provide pharmacy anticoagulation services
• Demonstrate proficiency in clinical microbiology and infectious disease pharmacology and assist with the Antimicrobial Management Program
• Register with Texas State Board of Pharmacy as a preceptor
• Participate in training of PGY1 pharmacy residents, new pharmacists, pharmacy students and technicians

Ideal Candidate:

• Registered Pharmacist in the state of Texas
• Must be fully COVID-19 vaccinated, no exceptions
• Must have recent hospital experience
• Full time availability
