Location: Houston, TX
Schedule: Variable days and afternoons, 8-hour shifts, rotating weekends
Job Responsibilities:
- Evaluate the appropriateness and effectiveness of patient’s active medication therapy
- Recognize untreated medical issues that could be improved or resolved with appropriate medication therapy
- Monitor patient progress for regimen effectiveness and safety
- Communicate effectively with physicians, nurses, and other healthcare providers to provide clinical pharmacy services and direct patient care activities
- Provide general clinical services such as pharmacokinetic based dosing, anticoagulant monitoring, pain management, nutrition support, IV to PO, renal dosing and other programs as defined by pharmacy leadership
- Demonstrate proficiency in clinical pharmacokinetic concepts and independently provide clinical pharmacokinetic services
- Demonstrate proficiency in anticoagulation therapy and independently provide pharmacy anticoagulation services
- Demonstrate proficiency in clinical microbiology and infectious disease pharmacology and assist with the Antimicrobial Management Program
- Register with Texas State Board of Pharmacy as a preceptor
- Participate in training of PGY1 pharmacy residents, new pharmacists, pharmacy students and technicians
Ideal Candidate:
- Registered Pharmacist in the state of Texas
- Must be fully COVID-19 vaccinated, no exceptions
- Must have recent hospital experience
- Full time availability
