An opportunity to provide skilled nursing care in a dynamic long-term care and rehab setting is available for experienced Licensed Practical Nurses (LPNs). This contract position offers the chance to support patients recovering from conditions such as stroke, amputation, neurological disorders, and post-joint replacement in a facility focused on both short-term and long-term care.

Key Responsibilities:

  • Deliver comprehensive nursing care including trach management, Foley catheter care, wound care, dressing changes, IV starts/management, and tube feedings within your scope of practice.
  • Collaborate with nursing leadership, therapy teams, case management, and healthcare technicians to coordinate patient care.
  • Utilize EMR systems (EPIC), manage medication dispensing via Omnicell, and operate IV pumps (Alaris).
  • Participate in floating assignments to various regional rehab locations as needed, with travel time compensated.
  • Adhere to facility policies regarding admissions, patient ratios, and clinical documentation.
  • Maintain current BLS certification and follow all safety protocols.

Qualifications and Experience:

  • Minimum 18 months of experience in skilled nursing or long-term care rehabilitation.
  • Proficiency with IV therapy, wound care, trach management, and other critical nursing skills.
  • Comfortable working in fast-paced environments with diverse patient needs.
  • Ability to work a rotating schedule including weekend shifts and holiday coverage.
  • Applicants should have a readiness to float within an established rehab region when required.

Schedule and Work Environment:

  • 12-hour day shifts from 7:00 AM to 7:00 PM, 36 hours per week.
  • Weekend rotation every other weekend.
  • No call responsibilities; holiday coverage approximately 75% of holidays.
  • Free parking and supportive nursing leadership.
  • Scrub color code: LPNs wear Ceil Blue.
