Providing compassionate home health care in the scenic Franklin and Macon County area, this role offers the opportunity to make a meaningful difference in patients’ lives. Serving approximately 5-6 patients daily, you will bring your home health and hospice nursing expertise to support individuals in their own homes, fostering comfort and independence.

Qualifications and Desired Experience:

  • Current Registered Nurse (RN) licensure with proven home health and hospice nursing experience
  • Familiarity with home health documentation systems, preferably HCHB
  • Reliable vehicle for patient visits throughout Franklin/Macon County
  • Ability to maintain professional appearance: navy or navy/white scrubs required for nursing staff
  • Strong interpersonal skills and commitment to patient-centered care

Responsibilities:

  • Deliver skilled nursing care to home health and hospice patients, adhering to individual care plans
  • Document patient information accurately using the HCHB electronic system
  • Collaborate with interdisciplinary teams to coordinate patient care and improve outcomes
  • Ensure compliance with all agency policies, safety standards, and infection control protocols
  • Maintain schedules Monday through Friday during daytime hours to support continuity of care
