Key Responsibilities:
- Screen, evaluate, and rehabilitate patients with physical functioning disorders or wounds using standardized assessments and evidence-based practices.
- Administer rehabilitation procedures such as manual techniques, therapeutic exercises, ambulation, modalities, wound care, and assistive device training.
- Continuously assess patient progress and adjust treatments accordingly, documenting all interventions and outcomes in compliance with regulations.
- Collaborate effectively with supervisors, healthcare team members, patients, families, and caregivers to provide consultation, counseling, and discharge planning.
