Responsibilities:
- Collect patient data from families and medical records, adhering to adolescent, adult, or geriatric communication principles
- Complete timely admission assessments and Braden scale evaluations
- Develop and revise comprehensive plans of care reflecting physical, psychosocial, and educational patient needs
- Continually reassess patient condition and coordinate nursing care prioritization and delegation
- Document patient responses to interventions, medications, and education accurately and in accordance with policy
- Assess teaching needs and deliver age-appropriate education materials to patients and families
- Initiate and implement discharge planning from admission through hospital stay, collaborating with interdisciplinary teams
- Accurately transcribe and clarify physician orders, and manage telemetry monitoring including arrhythmic parameter adjustments and cardiac event protocols
