Pharmacist Job - Charleston, WV

Inpatient Pharmacist Position in the Charleston, WV
250 bed community hospital, part of larger area health system.
Variable day/afternoon shifts.

Essential Duties:

• Reviews, processes, and dispenses physician medication orders and prescriptions
• Prepares and supervises the preparation IV admixtures and chemotherapy
• Provides therapeutic drug monitoring consultation, including determination of appropriate loading dose, maintenance dose, and predicted blood levels, based on patient specific parameters and disease process.
• Maintains routine workflow, integrates non-routine work and relays information as necessary to the oncoming shift.
• Consistently monitors and follows-up with unresolved medication order issues
• Monitors lab orders, department messages and follows up as needed
• Properly documents clinical interventions in the EHR system using provided guidelines.
• Knowledge and experience with Pyxis and other electronic Med stations
• Monitors and supervises Technicians activity, checks all medications prior to being filled into Med Stations
• Maintains all emergency drugs during shift

Qualifications, Skills, and Experience:

• BS in Pharmacy or PharmD
• 1 year of experience in an inpatient pharmacy 
• Active WV Pharmacist License.

Comp: starting at $70.00/hour total
