Apply NowThis opportunity is seeking experienced oncology and hematology outpatient pharmacists to join a respected medical center in Arlington Heights, IL. Schedule:
- Monday through Friday, day shifts
Key Responsibilities:
- Participate in interdisciplinary rounds for malignant hematology, medical oncology, gynecologic oncology, and stem cell transplant teams to optimize medication therapy.
- Review chemotherapy and other medication orders for appropriate dosing, interactions, and effectiveness in outpatient care.
- Reconcile home medications and clarify orders with prescribers, documenting all changes in the electronic medical record.
- Provide counseling to patients and caregivers on medication adherence, side effects, and therapy management.
- Conduct pharmacokinetic consults for medications such as vancomycin and aminoglycosides, preparing detailed notes.
- Monitor and report adverse drug reactions and medication errors promptly.
- Support nursing staff by being available to answer medication-related questions.
- Assist with chemotherapy desensitization protocols when needed.
Desired Qualifications:
- Bachelor of Science in Pharmacy.
- Current Illinois Pharmacist License or eligibility to obtain one.
- Completion of Post Graduate ASHP Year Two Pharmacy Residency in Hematology/Oncology preferred.
- Board Certification in Oncology Pharmacy Specialty (BCOP) required within 18 months of hire.
- BLS Certification.
- Proficient knowledge of intravenous admixture services, chemotherapy, and cellular therapies.
- Strong problem-solving skills, independence, and ability to collaborate effectively.
