Full-Time Clinical Pharmacist Job - IN

Soliant is searching for a committed and seasoned clinical pharmacist with an active Indiana pharmacy license to become a valuable member of our growing team. This opportunity is a direct hire position, and the chosen candidate will assume a pivotal role within our hospital pharmacy department.

Responsibilities:

• Provide comprehensive pharmaceutical care to patients in collaboration with other healthcare professionals.
• Ensure safe and effective medication use by reviewing and verifying physician orders, dispensing medications, and providing drug information to healthcare staff and patients.
• Participate in medication management initiatives, including medication reconciliation, therapeutic drug monitoring, and patient education.
• Contribute to the development and implementation of pharmacy policies and procedures to maintain compliance with regulatory standards.
• Serve as a resource for drug information and clinical guidance to healthcare providers and patients.
• Participate in multidisciplinary rounds and collaborate with other healthcare professionals to optimize patient outcomes.

Requirements:

• Pharmacy degree from an ACPE-accredited institution.
• Completion of a PGY1 pharmacy residency program or equivalent hospital practice experience.
• Active pharmacy licensure, or eligibility, in the state of Indiana.
• Strong clinical knowledge and understanding of hospital pharmacy operations.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a fast-paced environment and prioritize tasks efficiently.
• Commitment to providing exceptional patient care and promoting medication safety.
• Availability to work day shifts on a full-time basis.
