Apply NowA highly skilled clinical pharmacist opportunity is available, specializing in oncology and hematology outpatient care. This direct hire position offers a chance to contribute to a multidisciplinary healthcare team utilizing EPIC EMR in a dynamic, patient-focused setting.
Location:

Schedule:
- Monday through Friday, day shifts in an outpatient setting
Key Responsibilities:
- Participate in interdisciplinary rounds for malignant hematology, medical oncology, gynecologic oncology, and stem cell transplant (SCT) teams.
- Review and verify chemotherapy orders and medication regimens to ensure safety, efficacy, and cost-effectiveness.
- Reconcile home medications for admitted patients and collaborate with healthcare providers on order clarifications.
- Provide comprehensive patient and family counseling on medication adherence and potential adverse effects.
- Conduct pharmacokinetic consultations, particularly for vancomycin and aminoglycosides, and document accordingly.
- Monitor, document, and report adverse drug reactions and medication errors.
- Support nursing staff with medication order questions and verify medication orders for assigned units.
- Assist in chemotherapy desensitizations as needed and manage IV to PO medication conversions.
- Provide coverage for evening shifts as departmental needs arise.
Desired Qualifications:
- Bachelor of Science in Pharmacy degree
- Licensed or eligible for licensure as a pharmacist in Illinois
- Completion of a Post Graduate ASHP Year Two Pharmacy Residency in Hematology/Oncology preferred
- Board Certification in Oncology Pharmacy Specialty (BCOP) within 18 months of hire is required
- BLS Certification
- Experience with intravenous admixture services, drug therapy including chemotherapy and cellular therapies
- Strong problem-solving skills and ability to work independently and collaboratively
