Per Diem LTC Staff Pharmacist | Baraboo, Wisconsin

A Long Term Care facility in Mauston, Wisconsin, is seeking a Per Diem relief pharmacist to provide essential coverage during vacation periods and assist with workload fluctuations throughout 2025. This opportunity suits an experienced pharmacist ready to support a small, dedicated team at a well-established LTC pharmacy operating with a single pharmacist owner.

Location:

• Mauston, WI (121 E. State Street)
• Pharmacy operating hours:
  • Monday to Friday: 8:00 AM – 5:00 PM
  • Saturday: 8:00 AM – 1:00 PM
  • Sunday: Closed

Responsibilities:

• Accurate medication dispensing and packaging for long-term care patients
• Managing inventory and coordinating timely delivery of pharmaceuticals
• Utilizing pharmacy technology to maintain high quality and safety standards
• Ensuring full regulatory compliance within the pharmacy setting
• Facilitating clear communication with patients and healthcare providers

Desired Experience & Qualifications:

• Active Wisconsin pharmacist license is required
• Previous experience in long-term care pharmacy settings
• Familiarity with Framework pharmacy software is a significant advantage; training is available for the right candidate
• Strong attention to detail and ability to work independently

Position Details:

• Per Diem schedule with an initial assignment from March 23 through April 1, 2026
• Flexible availability to cover additional dates throughout 2025
• Mileage and hotel accommodations covered by the client
