Per Diem Retail Staff Pharmacist | Irwin, Pennsylvania

This per diem opportunity is for experienced pharmacists seeking to work within a specialty retail pharmacy environment focused on complex patient care. The role involves providing personalized medication consultation and management for patients who are typically managing multiple medications daily. Strong communication and disease-state management skills are essential to support patients effectively.

Responsibilities include:

• Providing comprehensive medication review and counseling for patients with chronic, advanced disease states such as diabetes, hypertension, and multiple sclerosis
• Consulting with patients who may have low health literacy, demonstrating patience, compassion, and exceptional communication skills
• Utilizing Pioneer Rx software to manage prescriptions and documentation accurately
• Collaborating with the healthcare team to ensure optimal medication therapy and adherence
• Supporting a patient population primarily consisting of Medicare/Medicaid and low-income individuals, ensuring they understand their treatment plans

Qualifications and Desired Experience:

• Valid pharmacy license in PA
• Strong background in retail pharmacy, with specialty pharmacy experience highly preferred
• Proven ability to manage and consult on complex medication regimens involving 8-15 medications daily
• Expertise in disease state management and patient consultation, particularly for chronic illnesses and advanced disease states
• Experience with Pioneer Rx software is a plus
• Compassionate approach to working with diverse patient populations, especially those with low literacy levels
