Qualifications & Desired Experience

  • Active RN license with Nursys verification on profile
  • Minimum of 2 years’ experience in Case Management
  • BLS certification required
  • Care Management certification (AMC or CCM) preferred
  • Degree accreditation must be documented on profile or minimum 5 years case management experience
  • Flexibility to float to sister campuses as needed

Schedule & Hours

  • Weekend commitment: one weekend shift per month

Responsibilities

  • Evaluate patient needs and develop individualized care plans
  • Coordinate services among healthcare providers and community resources
  • Assist patients and families in navigating the healthcare system
  • Monitor patient progress and adjust care plans accordingly
  • Collaborate with a multidisciplinary team to ensure seamless patient transitions
