An opportunity is available for an experienced Registered Nurse specializing in Case Management to support patient care coordination in Lehigh Acres, FL. This contract role is ideal for a professional with strong clinical expertise and the ability to manage complex cases efficiently.

Key Responsibilities:

• Conduct comprehensive assessments to develop and implement individualized care plans
• Coordinate hospital discharges and transitions of care to ensure continuity
• Collaborate with interdisciplinary teams, patients, and families to optimize outcomes
• Manage utilization review and resource allocation in compliance with regulatory standards
• Provide patient education and support to foster engagement and adherence

Qualifications and Experience:

• Current RN license with Nursys validation on profile
• Minimum of 2 years’ experience in Case Management
• BLS certification required
• Care Management certification (AMCC or CCM) preferred
• Degree accreditation must be documented on profile or have 5 years of professional experience
• Ability to float to sister campuses as needed
• Flexibility to cover at least one weekend shift per month
