The patient population ranges from newborns to centenarians, requiring a versatile and adaptive approach. Collaborate closely with patients, families, physicians, and interdisciplinary health professionals to assess needs and coordinate comprehensive care plans. Effective management of technical, environmental, and financial resources is essential in delivering quality care.
Qualifications and Desired Experience:
- Licensed Occupational Therapist registered with the Board of Registration in Massachusetts
- Minimum of 2 years’ experience in acute care, rehabilitation, or home care settings
- At least 1-year certification in home care occupational therapy
- Proficiency with various computer software; familiarity with handheld patient care devices preferred
- Strong customer service skills with the ability to work effectively with diverse populations
- Excellent verbal, written, and electronic communication skills
- Ability to prioritize tasks and adapt to evolving patient needs and clinical situations
- Ability to travel within the geographic service area as required
Responsibilities:
- Conduct comprehensive occupational therapy assessments in patients' homes
- Develop and implement customized care plans in collaboration with multidisciplinary teams
- Monitor patient progress and adjust treatment strategies accordingly
- Educate patients and families on therapeutic techniques and health maintenance
