Provide personalized occupational therapy care directly in patients’ homes as a key member of a compassionate Home Health team. This contract opportunity is based in Clarion, IA, offering you the chance to make a meaningful impact by helping patients regain independence and improve their quality of life through individualized therapy plans.

Key Responsibilities:

  • Conduct home visits for patients requiring occupational therapy services, managing ongoing interdisciplinary assessments.
  • Evaluate patients’ functional levels using appropriate clinical tests and assessments.
  • Develop and implement specific occupational therapy procedures aligned with physician treatment plans.
  • Create and oversee treatment plans including environmental modifications tailored to patient needs.
  • Manage the application of therapeutic modalities, prosthetic devices, and specialized equipment as needed.
  • Collaborate closely with physicians and the care team to monitor patient progress and adjust treatments accordingly.

Desired Qualifications:

  • Licensed Occupational Therapist with current credentials.
  • Proven experience in home health or hospital occupational therapy settings.
  • Strong clinical assessment skills and proficiency in creating functional therapy programs.
  • Excellent communication skills to build effective therapeutic relationships.
  • Ability to work independently and manage a caseload in a home-based environment.
