Key Responsibilities:
- Conduct home visits for patients requiring occupational therapy services, managing ongoing interdisciplinary assessments.
- Evaluate patients’ functional levels using appropriate clinical tests and assessments.
- Develop and implement specific occupational therapy procedures aligned with physician treatment plans.
- Create and oversee treatment plans including environmental modifications tailored to patient needs.
- Manage the application of therapeutic modalities, prosthetic devices, and specialized equipment as needed.
- Collaborate closely with physicians and the care team to monitor patient progress and adjust treatments accordingly.
Desired Qualifications:
- Licensed Occupational Therapist with current credentials.
- Proven experience in home health or hospital occupational therapy settings.
- Strong clinical assessment skills and proficiency in creating functional therapy programs.
