An experienced Occupational Therapist is sought for a home health contract assignment in the Nipomo, CA area. This role involves delivering skilled therapy services primarily within a designated geographical region, focusing on patients with orthopedic, stroke, joint, and spinal injury diagnoses, among others. Candidates with a strong background in home health care and familiarity with OASIS documentation will find this opportunity well-suited to their skills.
Key Responsibilities:
- Provide occupational therapy services in patients' homes, adhering to best practices and regulatory requirements.
- Conduct patient evaluations, develop care plans, and document progress using the Cerner EMR system.
- Collaborate with rehab aides, OTA/PTA staff, and interdisciplinary teams to optimize patient outcomes.
- Maintain compliance with government-mandated forms such as OASIS.
- Cover assigned geographical areas with flexibility in scheduling; primarily Monday through Friday, 8 am to 5 pm, with weekend coverage every third weekend and holiday availability as needed.
- Participate in a floor-specific orientation lasting approximately 1.5 to 2 weeks.
- Maintain professional business attire with a white lab coat; jeans are not permitted.
Qualifications and Desired Experience:
- Active California Occupational Therapy license.
- Minimum of two years of professional experience.
- Valid Basic Life Support (BLS) certification.
- Experience with home health care preferred.
- Knowledge and practical use of OASIS documentation.
- Comfort working within a home setting and flexibility to cover multiple patient locations.
