Deliver occupational therapy services in patients’ homes as part of a dedicated Home Health team. This role focuses on creating personalized therapy plans that promote independence, enhance quality of life, and support patients in achieving their health goals. Working closely with physicians and interdisciplinary care teams, you will apply your clinical skills and compassionate care to deliver hands-on treatment, patient education, and support grounded in best practices.
Responsibilities:
- Conduct home visits to provide occupational therapy services and perform ongoing interdisciplinary patient assessments.
- Evaluate patients’ functional levels using appropriate assessment tools and clinical judgement.
- Develop and implement individualized occupational therapy treatment plans aligned with physician directives and patient needs.
- Recommend and apply environmental modifications to support patient safety and function.
- Manage the use of therapeutic modalities, prosthetic devices, and assistive appliances as part of treatment.
- Collaborate with healthcare providers and caregivers to optimize patient outcomes and continuity of care.
Qualifications / Desired Experience:
- Valid Occupational Therapist licensure as required by state regulations.
- Experience delivering occupational therapy in a home health or similar setting preferred.
- Strong clinical assessment and treatment planning skills.
- Excellent communication and interpersonal abilities to engage patients and families.
- Ability to work independently and manage a dynamic caseload.
