Location
Based in Camden, NJ, this position offers opportunities to serve patients in Mercer County and North Burlington County. Responsibilities

  • Conduct comprehensive patient evaluations and develop individualized therapy plans.
  • Deliver in-home physical therapy aimed at improving mobility, function, and overall well-being.
  • Educate patients and caregivers on tailored exercises and home care strategies.
  • Collaborate closely with physicians, nurses, and case managers to optimize patient outcomes.
  • Maintain clear, accurate, and timely documentation of all therapy sessions.

Qualifications

  • Graduate from an accredited Physical Therapy program.
  • Licensed Physical Therapist in New Jersey.
  • Current AHA BLS Certification.
  • Minimum of 2 years’ experience working with adult geriatrics.
  • Proven experience in home health and senior care settings.
  • Strong communication, documentation, and problem-solving skills.
  • Reliable transportation to accommodate home visits.

Preferred

  • Experience in pelvic floor therapy.
  • Completion of Herman & Wallace Level 1A training or equivalent.

Schedule

  • Monday through Friday, 8:00 AM to 4:30 PM
  • No on-call duties or weekend work required

Physical Demands

  • Frequent standing, walking, and driving.
  • Occasional lifting up to 50 lbs., bending, and reaching.
