Physical Therapist - Conway, NH

A skilled Physical Therapist is sought for a contract position in a hospital setting located in Conway, NH. This role involves performing detailed physical therapy evaluations, developing individualized treatment plans, and implementing therapies that restore function, alleviate pain, and prevent disability in patients recovering from various conditions such as orthopedic, neurological issues, and general deconditioning. Care provided will align with prescribed Plans of Care and standards set by the American Physical Therapy Association.

Responsibilities:

• Conduct comprehensive physical therapy evaluations for inpatient and outpatient populations
• Develop and implement effective treatment plans tailored to individual patient needs
• Document patient progress and update care plans accordingly using EPIC EMR system
• Collaborate with interdisciplinary teams to optimize patient outcomes
• Promote safe and professional appearance adhering to dress code guidelines

Qualifications & Experience:

• Graduate of an APTA accredited Physical Therapist program
• Current, permanent NH Physical Therapist license required
• BLS certification current or obtained within 30 days of hire
• Minimum of 2 years of experience preferred, with outpatient and/or inpatient Physical Therapy exposure highly desirable
• Experience with EPIC EMR is a plus

Schedule & Hours:

• Day shift schedule
• 36 guaranteed hours per week
