Provide specialized outpatient physical therapy services in a multidisciplinary setting focused on patient-centered care and functional recovery. This contract role involves evaluating patients, developing personalized treatment plans, and guiding therapy services to support effective return to community activities. The position includes responsibilities for patient education and collaboration with patients’ families, students, and healthcare team members.
Key Responsibilities:
- Conduct comprehensive patient evaluations and develop treatment plans aligned with physician direction and patient goals.
- Oversee and direct discipline-specific therapy services for assigned patients.
- Educate patients, families, students, and colleagues to promote understanding and engagement in therapy.
- Ensure documentation accuracy, including goal writing, progress notes, letters of medical necessity, and charge entry.
- Adhere to compliance with organizational policies, state practice acts, and professional ethical standards.
- Participate in student clinical education by providing supervision and evaluation.
- Engage in continuous professional development and contribute to performance improvement initiatives.
- Promote and maintain patient and employee safety by following safety protocols and organizational guidelines.
Desired Experience and Qualifications:
- Valid physical therapy licensure in good standing.
