Primary Responsibilities:
- Conduct comprehensive evaluations and develop tailored treatment plans for inpatients and transition-of-care needs
- Provide therapeutic interventions to restore mobility, minimize pain, and optimize functional independence
- Collaborate with nurses, physicians, and rehabilitation colleagues to coordinate and deliver comprehensive patient care
- Educate patients and their families on home exercise programs, injury prevention, and progress tracking
- Maintain accurate patient records and contribute to ongoing quality improvement initiatives
