Providing specialized physical therapy services within patients’ homes in Mercer County and North Burlington County, this role focuses on empowering elderly individuals to maintain their independence and improve quality of life. The position requires conducting thorough patient evaluations, designing personalized treatment plans, and collaborating closely with a committed healthcare team to ensure comprehensive care.

Key Responsibilities: • Perform detailed patient assessments to develop targeted therapy plans. • Deliver therapeutic interventions to enhance mobility, functional ability, and well-being. • Educate patients and caregivers on exercises and home care strategies. • Coordinate care with physicians, nurses, and case managers to optimize outcomes. • Maintain precise and timely documentation in compliance with regulations.

Qualifications: • Graduate from an accredited Physical Therapy program. • Licensed Physical Therapist in New Jersey. • AHA Basic Life Support (BLS) Certification mandatory. • Minimum of 2 years’ experience working with adult geriatric populations. • Proven background in home health and senior care therapy. • Strong skills in communication, documentation, and clinical problem-solving. • Reliable transportation essential for home visits.

Preferred Qualifications: • Specialized experience in pelvic floor therapy. • Herman & Wallace Level 1A training or equivalent certification.

Physical Demands & Work Environment: This role involves frequent standing, walking, and driving, with occasional lifting of up to 50 lbs., bending, and reaching. Schedules are Monday through Friday, 8:00 AM to 4:30 PM, with no weekend or on-call duties required.
