Responsibilities:
- Evaluate and develop individualized physical therapy treatment plans in compliance with physician orders and state regulations
- Provide therapy services across inpatient, swing bed, outpatient, and skilled nursing units
- Document patient progress accurately within EMR systems
- Collaborate with multidisciplinary teams to optimize patient outcomes
- Maintain adherence to all relevant clinical and regulatory standards
Qualifications:
- Bachelor’s, Master’s, or Doctor of Physical Therapy (DPT) from an accredited institution
- Active California Physical Therapy License in good standing
- Successful completion of the National Physical Therapy Examination (NPTE)
- Current AHA Basic Life Support (BLS) certification
- Minimum of 1 year clinical experience in acute care, skilled nursing, or outpatient settings
- Strong communication, clinical reasoning, and documentation capabilities
- Proficiency with electronic medical records and computerized systems
- Dedication to ongoing professional development and patient-centered care
