Responsibilities:

• Evaluate and develop individualized physical therapy treatment plans in compliance with physician orders and state regulations
• Provide therapy services across inpatient, swing bed, outpatient, and skilled nursing units
• Document patient progress accurately within EMR systems
• Collaborate with multidisciplinary teams to optimize patient outcomes
• Maintain adherence to all relevant clinical and regulatory standards

Qualifications:

• Bachelor’s, Master’s, or Doctor of Physical Therapy (DPT) from an accredited institution
• Active California Physical Therapy License in good standing
• Successful completion of the National Physical Therapy Examination (NPTE)
• Current AHA Basic Life Support (BLS) certification
• Minimum of 1 year clinical experience in acute care, skilled nursing, or outpatient settings
• Strong communication, clinical reasoning, and documentation capabilities
• Proficiency with electronic medical records and computerized systems
• Dedication to ongoing professional development and patient-centered care
