A licensed Physical Therapist is needed for a contract position providing rehabilitative care to home health and hospice patients. This opportunity requires recent travel experience and offers the chance to make a meaningful impact on patients’ quality of life through coordinated therapy services in their own homes. Experience with Cerner and working within a hospital or home health setting is highly valued.

Location:
This role is based near Surrey, ND, serving communities within a 50-mile radius. The position involves traveling to patients’ homes, making a reliable driver’s license and vehicle essential. The surrounding area offers a combination of small-town charm and access to local amenities, providing a comfortable lifestyle for healthcare professionals who enjoy traveling for patient care.

Responsibilities:

  • Deliver rehabilitative physical therapy services to home health and hospice patients, emphasizing safety and education.
  • Develop and implement treatment plans focused on rehabilitation and preventative care under medical supervision.
  • Assess individual and family health needs, identifying potential hazards and coordinating care accordingly.
  • Collaborate with agency personnel and community resources to ensure comprehensive patient support.
  • Maintain documentation and supervise therapy interventions, especially for geriatric populations.

Desired Experience & Qualifications:

  • Licensed Physical Therapist in North Dakota.
  • Minimum of 2 years’ experience as a Physical Therapist preferred.
  • Recent travel experience required, with a current driver’s license and reliable vehicle.
  • Strong preference for candidates with Cerner electronic health record experience.
  • Ability to complete skills checklists and licensure verification (including full licensure reports if applicable).

Schedule:

  • 40 hours per week, Monday through Friday (five 8-hour days).
