A licensed Physical Therapist is needed for a contract position providing rehabilitative care to home health and hospice patients. Location:
This role is based near Surrey, ND, serving communities within a 50-mile radius. The position involves traveling to patients’ homes, making a reliable driver’s license and vehicle essential. Responsibilities:
- Deliver rehabilitative physical therapy services to home health and hospice patients, emphasizing safety and education.
- Develop and implement treatment plans focused on rehabilitation and preventative care under medical supervision.
- Assess individual and family health needs, identifying potential hazards and coordinating care accordingly.
- Collaborate with agency personnel and community resources to ensure comprehensive patient support.
- Maintain documentation and supervise therapy interventions, especially for geriatric populations.
Desired Experience & Qualifications:
- Licensed Physical Therapist in North Dakota.
- Minimum of 2 years’ experience as a Physical Therapist preferred.
- Recent travel experience required, with a current driver’s license and reliable vehicle.
- Strong preference for candidates with Cerner electronic health record experience.
- Ability to complete skills checklists and licensure verification (including full licensure reports if applicable).
Schedule:
- 40 hours per week, Monday through Friday (five 8-hour days).
