Provide expert physical therapy services in patients’ homes as part of a specialized program dedicated to supporting elderly individuals in maintaining their independence and quality of life. This contract opportunity requires visiting clients within Mercer County and North Burlington County, delivering high-quality care through personalized treatment plans.
Responsibilities:
- Perform comprehensive patient evaluations to assess mobility, function, and therapeutic needs
- Develop and implement individualized treatment plans tailored to each patient’s goals
- Educate patients and caregivers on exercises and home-based care techniques to enhance recovery
- Collaborate with physicians, nurses, and case managers to coordinate patient care
- Maintain accurate and timely clinical documentation in compliance with program standards
- Travel regularly to patients' homes within assigned counties; reliable transportation is essential
Qualifications:
- Graduate of an accredited Physical Therapy program
- Licensed Physical Therapist in New Jersey
- Current AHA Basic Life Support (BLS) certification
- Minimum two years’ experience working with adult geriatric populations
- Proven experience in home health and senior care therapy settings
- Strong communication, documentation, and problem-solving abilities
- Reliable transportation for home visits is mandatory
Preferred Qualifications:
- Experience and training in pelvic floor therapy, such as Herman & Wallace Level 1A or equivalent
Schedule:
- Monday through Friday, 8:00 AM to 4:30 PM
- No weekend or on-call duties required
The role involves regular standing, walking, driving, and occasional lifting (up to 50 lbs.), bending, and reaching.
