Provide expert physical therapy services in patients’ homes as part of a specialized program dedicated to supporting elderly individuals in maintaining their independence and quality of life. This contract opportunity requires visiting clients within Mercer County and North Burlington County, delivering high-quality care through personalized treatment plans.

Responsibilities:

  • Perform comprehensive patient evaluations to assess mobility, function, and therapeutic needs
  • Develop and implement individualized treatment plans tailored to each patient’s goals
  • Educate patients and caregivers on exercises and home-based care techniques to enhance recovery
  • Collaborate with physicians, nurses, and case managers to coordinate patient care
  • Maintain accurate and timely clinical documentation in compliance with program standards
  • Travel regularly to patients' homes within assigned counties; reliable transportation is essential

Qualifications:

  • Graduate of an accredited Physical Therapy program
  • Licensed Physical Therapist in New Jersey
  • Current AHA Basic Life Support (BLS) certification
  • Minimum two years’ experience working with adult geriatric populations
  • Proven experience in home health and senior care therapy settings
  • Strong communication, documentation, and problem-solving abilities
  • Reliable transportation for home visits is mandatory

Preferred Qualifications:

  • Experience and training in pelvic floor therapy, such as Herman & Wallace Level 1A or equivalent

Schedule:

  • Monday through Friday, 8:00 AM to 4:30 PM
  • No weekend or on-call duties required

The role involves regular standing, walking, driving, and occasional lifting (up to 50 lbs.), bending, and reaching.
