Physical Therapist | Velva, North Dakota

A licensed Physical Therapist is needed to provide specialized rehabilitative services in a home health and hospice setting. This role requires delivering comprehensive therapy to patients in their homes, emphasizing treatment, rehabilitation, and safety, particularly for the geriatric population. Qualifications & Desired Experience:

• Licensed Physical Therapist in North Dakota required
• Minimum of 2 years of recent experience as a Physical Therapist preferred
• Valid driver’s license and reliable vehicle to travel to patients’ homes
• Experience with Cerner electronic health records system strongly preferred
• Recent travel experience highly valued
• Ability to engage with home health and hospice patient populations, especially geriatrics

Responsibilities:

• Deliver rehabilitative therapy services to home health and hospice patients
• Develop and implement coordinated therapy plans tailored to patient and family needs
• Assess health needs and potential hazards for individuals and families
• Collaborate with healthcare team members and community services for comprehensive care
• Ensure all treatments comply with medical directions and agency standards
• Document patient care activities accurately under supervision of the clinical manager

Schedule & Type:

• Full-time, 40 hours per week (five 8-hour days)
• Contract position with flexible travel within the service area
