A licensed Physical Therapist is needed to provide specialized rehabilitative services in a home health and hospice setting. This role requires delivering comprehensive therapy to patients in their homes, emphasizing treatment, rehabilitation, and safety, particularly for the geriatric population. Qualifications & Desired Experience:
- Licensed Physical Therapist in North Dakota required
- Minimum of 2 years of recent experience as a Physical Therapist preferred
- Valid driver’s license and reliable vehicle to travel to patients’ homes
- Experience with Cerner electronic health records system strongly preferred
- Recent travel experience highly valued
- Ability to engage with home health and hospice patient populations, especially geriatrics
Responsibilities:
- Deliver rehabilitative therapy services to home health and hospice patients
- Develop and implement coordinated therapy plans tailored to patient and family needs
- Assess health needs and potential hazards for individuals and families
- Collaborate with healthcare team members and community services for comprehensive care
- Ensure all treatments comply with medical directions and agency standards
- Document patient care activities accurately under supervision of the clinical manager
Schedule & Type:
- Full-time, 40 hours per week (five 8-hour days)
- Contract position with flexible travel within the service area
