Provide expert speech-language pathology services in a long-term care setting, delivering patient-centered therapy to support rehabilitation and improve communication and swallowing functions. Qualifications and Desired Experience:
- Graduate of a college or university accredited by the American Speech and Hearing Association with a degree in Speech-Language Pathology
- Current New York State license as a Speech-Language Pathologist
- Eligible for or holds the Certificate of Clinical Competence (CCC) from the American Speech-Language-Hearing Association (ASHA)
- Proven experience working in long-term care (LTC) settings is required
- Strong communication skills to effectively collaborate with patients, medical staff, and multidisciplinary teams
- Excellent assessment skills to accurately evaluate patient potential and design individualized therapy plans
- Proficiency in Microsoft Office and Epic/EMR software
- Knowledge of the Patient-Driven Payment Model (PDPM) reimbursement system
Location: This role is based in Rome, New York, a historic city that combines a friendly community atmosphere with access to modern amenities. Located in the Mohawk Valley region, Rome offers a blend of rich heritage, outdoor recreational opportunities, and convenient access to larger metropolitan centers.
Responsibilities:
- Conduct comprehensive assessments to determine speech, language, and swallowing deficits
- Develop and implement individualized treatment plans tailored to patient needs and goals
- Collaborate with interdisciplinary teams to optimize patient care and rehabilitation outcomes
- Maintain accurate documentation and utilize EMR systems efficiently
- Communicate effectively with patients, families, and clinical staff regarding therapy goals and progress
Schedule:
- Shift hours: 7:00 AM to 3:30 PM
Additional Information: This contract role covers a 75-mile radius for travel, prioritizing local candidates within that area. Candidates living within 75 miles will receive a local rate. Prior long-term care experience in similar settings is mandatory.
