This contract role involves evaluating, screening, and rehabilitating patients with communication, cognitive, or swallowing disorders. The ideal candidate will utilize evidence-based practices to develop and adjust individualized treatment plans, ensuring all documentation meets regulatory and clinical standards.
Key Responsibilities:
- Conduct thorough assessments using standardized tools to diagnose speech and swallowing disorders.
- Develop, implement, and modify treatment plans based on patient progress and clinical findings.
- Provide counseling and consultation to patients, families, caregivers, and other healthcare providers.
- Communicate effectively with supervisors and multidisciplinary team members about patient status and treatment obstacles.
- Document all clinical data accurately in electronic health records and prepare comprehensive discharge summaries.
- Participate in patient care conferences, utilization reviews, and professional development meetings.
- Supervise and mentor Clinical Fellowship Year (CFY) therapists or student SLPs.
- Follow ASHA guidelines, state licensure requirements, professional ethics, and conduct codes.
- Coordinate the acquisition of necessary durable medical equipment and supplies.
Qualifications and Desired Experience:
- Active ASHA certification and current State Speech-Language Pathology licensure.
- Experience in hospital or skilled nursing settings preferred.
