An opportunity is available for a full-time skilled therapy professional specializing in social work within a healthcare setting. This role involves providing comprehensive psychosocial assessments and targeted interventions to patients and families enrolled in Home Health and Hospice programs. The position focuses on supporting clients through complex emotional and psychological processes related to terminal illness, adjustment to chronic conditions, family dynamics, and bereavement.

Responsibilities include:

  • Conducting in-depth psychosocial assessments to evaluate patient and family needs.
  • Delivering evidence-based interventions to support management of terminal illness and adjustment challenges.
  • Strengthening family and support systems to enhance coping mechanisms.
  • Facilitating conflict resolution related to chronic illness trajectories.
  • Providing compassionate death and dying support aimed at minimizing grief impact.
  • Collaborating with interdisciplinary teams to ensure cohesive, patient-centered care.

Qualifications and experience desired:

  • Master’s degree in Social Work from a program accredited by the Council on Social Work Education.
  • Minimum of two years’ experience as a clinical social worker, with direct clinical responsibilities.
  • Prior experience in Home Health and/or Hospice settings is strongly preferred.
  • Strong interpersonal skills with the ability to engage sensitively with patients and families during difficult times.

Location: Tahoe City, CA
Shift: 1st Shift
Duration: Contract, full-time in a school healthcare environment
