Transition of Care pharmacist at 400 bed hospital in the Milwaukee, WI area
This regional medical center is an affiliate of one of the Midwest's premier health system.

This Pharmacist position will have a focus on Transition of Care and patient discharge.

Duties:

• Will Function as a drug specialist and therapy consultant in promoting optimum medication use
• Compounds, packages, labels, and dispenses drugs and intravenous admixture solutions
• Reviews and confirms accuracy of work performed by pharmacy technicians
• Reviews prescribed drug therapy for appropriateness, effectiveness, and safety based on specific data such as age of patient, disease state, and clinical status.
• Performs quality assurance, drug use evaluation, and adverse reactions reporting activities;

Compensation: Starts at $65.00/hr  

Schedule: Day-shifts
Qualifications:

• PharmD.  

• WI Licensed Pharmacist.

• 1 + years of experience.
