Schedule:

• Five 8-hour shifts per week
• Rotating shifts—flexibility required

Key Responsibilities:

• Deliver compassionate home health care to patients in accordance with individualized care plans
• Collaborate with multidisciplinary teams to optimize patient outcomes
• Complete timely and accurate documentation for care episodes and patient progress
• Educate patients and families on health maintenance, safety, and disease prevention
• Adhere to best practices, all relevant state regulations, and patient care standards

Qualifications:

• Successful completion of a Licensed Practical Nursing Program approved by the State Board of Nursing
• Minimum of 2 years of experience as an LPN, preferably in home health
• Valid and unrestricted Pennsylvania LPN license
• Current Basic Life Support (BLS) certification
• Compliance with PA Act 31, PA Act 33, PA Act 34, and PA Act 73 requirements
