Qualifications & Requirements:

• Two years of relevant experience in home health or LPN roles
• Graduate of a Licensed Practical Nursing Program approved by the State Board of Nursing
• Active PA Licensed Practical Nurse (LPN) license
• Basic Life Support (BLS) certification
• Current approvals or clearances for PA Act 31, PA Act 33, PA Act 34, and PA Act 73

Key Responsibilities:

• Deliver holistic care to patients in home environments under the direction of RNs and physicians
• Conduct patient assessments, monitor vital signs, and document clinical data thoroughly
• Administer prescribed treatments and medications following established care plans
• Educate patients and families on treatment procedures, medication regimens, and preventive health
• Coordinate with the interdisciplinary team to ensure quality, personalized patient care
