Required Qualifications and Experience

• Graduation from an accredited Licensed Practical Nursing program as approved by the State Board of Nursing
• Minimum of 2 years of recent LPN experience in home health or healthcare settings
• Active Pennsylvania LPN license in good standing
• Current Basic Life Support (BLS) certification
• Compliance with PA Act 31, PA Act 33, PA Act 34, and PA Act 73

Key Responsibilities

• Deliver in-home patient care by assessing patient needs and implementing tailored nursing interventions
• Collaborate with multidisciplinary teams to develop and monitor patient care plans
• Accurately maintain comprehensive and timely documentation in accordance with regulations and policies
