Qualifications:

• Graduate of a Licensed Practical Nursing Program approved by the State Board of Nursing
• Minimum of 2 years of LPN experience, preferably with a background in home health care
• Active license and certifications in Pennsylvania:
  • Licensed Practical Nurse (LPN)
  • Basic Life Support (BLS)
  • PA Act 31, PA Act 33, PA Act 34, PA Act 73

Responsibilities:

• Deliver home health nursing care, ensuring patient safety and well-being
• Collaborate with interdisciplinary teams to implement individualized care plans
• Accurately document patient status, interventions, and responses
• Support patient and family education regarding ongoing care needs
• Adhere to all regulations and standards in clinical practice
