This travel assignment is ideal for an Occupational Therapist with at least three years of clinical experience, particularly in home health, SNF, or acute care. You'll work exclusively with adult patients, including geriatrics, post-stroke or joint replacement recovery, and those needing support with chronic conditions like COPD and CHF. Schedule/Hours:

• Monday through Friday, approximately 7:45 AM to 4:00 PM
• No weekends, on-call shifts, or holiday requirements

Qualifications:

• Minimum 3 years' experience as an Occupational Therapist (home health preferred, but SNF or acute care considered)
• Active WI OT license and BLS certification
• Bachelor’s degree or higher in Occupational Therapy
• Valid driver’s license, reliable vehicle, and current auto insurance
• Strong evaluation and treatment skills focused on activities of daily living and adaptive equipment

Responsibilities:

• Serve a caseload of six patients/points each day in their homes
• Conduct thorough evaluations and standard visits, tailoring care for adult home health patients
• Attend daily interdisciplinary team huddles (via phone)
• Document in EPIC electronic medical records system
