By collaborating within an interdisciplinary team of RNs and PTs, you will enhance your professional skills, learn innovative care practices, and gain valuable insight into effective leadership styles in home health. Schedule/Hours:

  • Monday – Friday, approximately 7:45 AM to 4:00 PM
  • No weekend, on-call, or holiday requirements
  • Flexible approval for requested time off (RTO)

Desired Qualifications:

  • Minimum 3 years' OT experience (SNF/home health/acute care—home health preferred)
  • Active Wisconsin OT license; BLS certification required
  • Bachelor’s degree in Occupational Therapy minimum
  • Valid driver’s license, reliable vehicle, and auto insurance
  • Comfort with general OT assessments, ADLs, adaptive equipment, and EMR systems (EPIC preferred)

Duties:

  • Provide home health OT care to adults, including post-acute, neurological, and chronic conditions
  • Conduct evaluations, develop care plans, and support patient independence
  • Engage daily in interdisciplinary telephonic team huddles
  • Manage a caseload of 6 patients/points per day
  • Document thoroughly in the EMR
