Qualifications & Preferred Experience:

• Master’s degree in Speech-Language Pathology
• Valid, current state licensure as an SLP
• At least one year of experience in home health or healthcare SLP roles
• Proficient in conducting assessments, crafting care plans, and documenting progress

• Ability to work independently while collaborating with multidisciplinary teams
• Reliable transportation and willingness to travel locally for home visits

Role Overview & Core Responsibilities:

• Conduct comprehensive speech, language, and swallowing evaluations for patients in their homes
• Develop and implement individualized treatment plans addressing communication, speech, voice, and feeding/swallowing disorders
• Educate patients, caregivers, and family members on therapeutic techniques and progress

• Maintain accurate, timely documentation consistent with regulatory and agency standards
• Monitor, reassess, and adapt interventions based on patient progress and goals
