Key Qualifications:
- Master’s degree in Speech-Language Pathology from an accredited program
- Current SLP license
- Prior experience in home health or healthcare settings preferred
- Strong communication, organization, and documentation skills
- Ability to develop customized care plans and work independently with minimal supervision
Responsibilities:
- Conduct comprehensive assessments of speech, language, cognitive-communication, and swallowing disorders
- Develop and implement individualized therapy treatment plans based on patient assessments
- Provide education and training to patients, caregivers, and family members to improve at-home care
- Maintain accurate clinical documentation in compliance with healthcare regulations
- Collaborate with referring physicians, nurses, and other healthcare professionals to ensure coordinated patient care
- Monitor, evaluate, and document patient progress, adapting plans as needed for optimal outcomes
