Key Qualifications:

• Current SLP license

Responsibilities:

• Conduct comprehensive assessments of speech, language, cognitive-communication, and swallowing disorders
• Develop and implement individualized therapy treatment plans based on patient assessments
• Provide education and training to patients, caregivers, and family members to improve at-home care
• Maintain accurate clinical documentation in compliance with healthcare regulations
• Collaborate with referring physicians, nurses, and other healthcare professionals to ensure coordinated patient care
• Monitor, evaluate, and document patient progress, adapting plans as needed for optimal outcomes
