As a Speech-Language Pathologist specializing in home health services, you will assess, diagnose, and design treatment plans to support patients with communication and swallowing disorders. Key Qualifications & Experience:

  • Valid and current Speech-Language Pathologist license
  • Prior experience in home health or similar healthcare setting strongly preferred
  • Expertise in evaluating and treating a range of speech, language, voice, and swallowing difficulties
  • Strong interpersonal skills for effectively communicating with patients, families, and the care team
  • Ability to work autonomously, demonstrating reliability and initiative
  • Strong documentation skills are essential

Role Responsibilities:

  • Conduct comprehensive assessments of patients in their homes
  • Develop and implement individualized treatment plans targeting communication, cognitive, and swallowing deficits
  • Educate and support families and caregivers regarding treatment approaches and at-home strategies
  • Collaborate with physicians, nurses, and other allied health professionals to ensure coordinated patient care
  • Maintain accurate and timely patient records in compliance with professional and regulatory standards
  • Monitor progress and adjust interventions as needed to achieve patient goals
