Key Qualifications:

  • Licensed Speech-Language Pathologist (SLP)
  • Previous experience in home health or similar healthcare environments preferred

Primary Responsibilities:

  • Conduct thorough speech, language, cognitive, and swallowing evaluations for home-based patients
  • Develop and execute evidence-based treatment plans tailored to each patient’s needs and goals
  • Document all visits and progress in accordance with regulatory and organizational standards
  • Educate patients, caregivers, and families on communication strategies and therapeutic techniques
  • Collaborate regularly with physicians, nurses, and other allied health professionals to coordinate comprehensive care
  • Monitor patient progress and modify care plans as required to achieve optimal outcomes
